Patient Admission Criteria

Patient must be under the care of a physician.

Home Bound Status:

Patient does not have to be confined in a bed or in a wheelchair to be considered as homebound.

Plan of Treatment:

Initially, a physician's verbal orders received by an RN are needed to start service. Then a copy of the signed, written plan of treatment will follow immediately.


The patient must require at least one of the following services:

• Skilled Nursing
• Social Services
• Occupational Therapy
• Chemo Therapy
• Physical Therapy
• Speech Therapy
• Enterostomal Therapy

The patient must reside geographically within the area encompassing the cities of:

Las Vegas
North Las Vegas
Henderson

  • Medicare
    • Medicare Eligibility
    To be eligible for Medicare coverage, patient must meet the following Medicare requirements:
  • Be a Medicare cardholder

  • Be confined to home
  • Require skilled nursing, physical therapy, or speech therapy services on an intermittent basis
  • Have a plan of treatment established by the personal physician
  • Be compliant
  • Individuals who are eligible to receive Medicare reimbursed home care visits can expect a professional assessment and evaluation of their needs. Intermittent visits may be available if Medicare guidelines are met.

    To receive services, patients must be:
  • Under the regular care of a physician
  • Homebound in a safe environment
  • Have a need for intermittent (part-time) skilled nursing care, physical or speech therapy
  • Additional services of occupational therapy, medical social work and home health aide assistance are available once skilled services are established.
